At Clarity Care, we're proud to serve those who served our country. As a registered Department of Veterans' Affairs (DVA) provider, we deliver professional community nursing services to veterans with Gold and White Cards across the Mandurah and Peel Region.
WHO WE SERVE
We provide care for veterans who: • Hold a DVA Gold Card or White Card • Reside in Mandurah or the Peel Region, Western Australia • Require skilled nursing care in their home • Have been referred by their GP, specialist, or hospital
OUR DVA NURSING SERVICES
WOUND CARE Expert management of surgical wounds, chronic wounds, leg ulcers, and pressure injuries. Our registered nurses provide specialized dressing changes and wound assessment to promote healing and prevent complications.
POST-SURGICAL CARE Smooth transition from hospital to home with comprehensive post-operative monitoring, wound care, and recovery support. We work closely with your surgical team to ensure optimal outcomes.
MEDICATION MANAGEMENT Safe medication administration including injections, infusions, and IV therapy. We also provide medication education and support for self-management.
DISEASE MANAGEMENT Ongoing support for diabetes, cardiac conditions, respiratory diseases, and other chronic health conditions. Our nurses monitor your health, provide education, and coordinate with your doctor.
CLINICAL ASSESSMENTS Comprehensive health assessments and monitoring including vital signs, general health checks, and disease-specific observations to detect changes early.
PALLIATIVE CARE Compassionate end-of-life care focused on comfort, dignity, and symptom management. We support both veterans and their families during this time.
CONTINENCE MANAGEMENT Professional support for continence issues including catheter care, continence assessment, and management strategies.
HEALTH EDUCATION Empowering you with knowledge and skills for self-management, disease prevention, and maintaining independence at home.

HOW IT WORKS STEP 1: REFERRAL Your doctor, specialist, or hospital discharge planner refers you to Clarity Care. Referrals can be made by phone, email, or fax. STEP 2: INITIAL CONTACT We'll contact you within 24 hours to introduce ourselves, discuss your needs, and arrange the first visit at a time convenient for you. STEP 3: FIRST VISIT A registered nurse will visit you at home to complete a comprehensive assessment, explain the care plan, and begin treatment if appropriate. STEP 4: ONGOING CARE We'll visit as often as needed - daily, weekly, or as per your care plan. Your nurse will keep your doctor informed of your progress. STEP 5: COMPLETION When you've achieved your health goals or no longer require nursing care, we'll provide a completion report to your doctor with any ongoing recommendations.
FREQUENTLY ASKED QUESTIONS Q: Am I eligible for DVA community nursing? A: If you hold a DVA Gold Card or White Card and require nursing care at home, you're likely eligible. Your doctor will determine if community nursing is appropriate for your needs. Q: How much will it cost me? A: Nothing. All our services are fully funded by DVA for eligible card holders. We submit all claims directly to DVA on your behalf. Q: How quickly can you start? A: We respond to all referrals within 48 hours. For urgent cases, we can often arrange service within 24 hours or even same-day. Q: What areas do you cover? A: We service all suburbs within Mandurah and the Peel Region, including Mandurah, Pinjarra, Waroona, Dwellingup, and surrounding areas. Q: Can I choose my visit times? A: Absolutely. We work around your schedule and preferences as much as possible. Just let us know your preferred times when we call to arrange the first visit. Q: Will I see the same nurse each time? A: We try to provide consistency wherever possible so you build a relationship with your nurse. However, for 7-day service, you may occasionally see different nurses from our team.
