Refer yourself, or refer someone you support. The form takes about five minutes. You do not need to have every detail ready — we will follow up on anything missing.

If you are a referrer, you get regular updates — with the participant’s consent.

Support hours are not office hours. Our office is open on weekdays, but support shifts can be rostered evenings, weekends and overnight by arrangement.

Why support coordinators refer to us
- AHPRA-registered clinical leadership. Registered nurses and mental health professionals supervise the work, not just coordinate it.
- 24-hour response to every referral. You are not left wondering whether it landed.
- Monthly progress reports. Written to meet clinical governance and NDIS reporting requirements.
- Complex psychosocial presentations welcome. Dual diagnosis, behaviours of concern, and participants other providers have declined.
- Small caseloads. Continuity of staff, which matters more than almost anything else in this work.
- Mandurah, Peel, South West and Perth South. Including the regional towns most providers will not travel to.

The information in this form includes health information, which the law treats as especially sensitive. We collect it so we can assess whether we can help and arrange the right support.
We only share it with people who need it to provide your support, and we ask for your consent before we share it with anyone else — including the person who referred you.
You can ask us what we hold about you, ask us to correct it, or ask us to delete it. Read our privacy policy or call and ask.
