Services
SIL, community nursing, psychosocial recovery coaching, positive behaviour support, daily living and community participation across Mandurah and the Peel Region.
What we do
Every service is delivered by, or supervised by, AHPRA-registered clinicians. If you are not sure which of these fits, call and we will work it out with you — including telling you if another provider is the better match.
Supported Independent Living
Your own home, your own routine, with trained staff there when you need them. We work out with you how much support is right — without taking over.
Good fit if: you want stable, long-term housing support with clinical oversight behind it.
Community Nursing
Wound care, medication support, continence support and chronic condition management in your own home.
Good fit if: you have health needs that require a registered nurse, and would rather not travel to get them met.
Psychosocial Recovery Coaching
Work alongside a qualified mental health professional to build coping strategies, reconnect socially, manage symptoms and navigate the systems around you.
Good fit if: you have psychosocial recovery coaching funding in your plan.
Positive Behaviour Support
Our registered behaviour support practitioner works with you to understand what is driving distress and build approaches that reduce it.
Good fit if: you need a behaviour support plan, or you are moving away from restrictive practices.
Assistance with Daily Life
Morning routines, meals, appointments, the weekly shop — support for the tasks that keep life running, built around growing your confidence.
Good fit if: you are building life skills or need a hand to keep things on track.
Assistance with Household Tasks
Cleaning, laundry and keeping your home the way you like it — done with you where you want to be involved, and for you where you do not.
Good fit if: maintaining your home has become harder than it used to be.
Social and Community Participation
Group activities, outings, skill-building and transport support, at the pace that works for you.
Good fit if: you want to get out more, meet people, or rebuild confidence in the community.
Psychosocial Functional Capacity Assessment
A structured look at what you can do, where things get hard, and what support would actually help — written up to support your plan review.
Good fit if: you need evidence for a plan review or reassessment.
Veterans
We are a registered DVA provider delivering community nursing to Gold and White Card holders across Mandurah and the Peel Region.
