You are carrying a full caseload. The last thing you need is a provider who needs managing. Registered nurses and mental health professionals supervise our work — not just coordinate it.

Monthly progress reports

Written to meet clinical governance and NDIS reporting requirements, so you can drop them straight into a plan review.

You talk to a clinician

When you call about a participant, you reach the registered practitioner working with them. Not a call centre, not an account manager.

Small caseloads, consistent staff

Continuity of support workers matters more than almost anything else in this work, and it is the first thing that breaks when a provider grows too fast. We roster to protect it.

Honest capacity answers

If we cannot take a referral, or we are not the right fit, we will say so straight away and point you to who can. You will never be left holding a maybe.

Regional reach

Mandurah, the Peel Region, the South West and southern Perth — including the regional towns most providers will not travel to.

Participants we can take

We work with people living with disability, mental health conditions and psychosocial disability, and with DVA Gold and White Card holders.

There is no age limit on our services. DVA community nursing regularly supports veterans well over 65, and positive behaviour support is available to children and young people as well as adults.

We are comfortable with:

  • Psychosocial disability, including complex and long-standing presentations
  • Dual diagnosis
  • Behaviours of concern, and participants moving away from restrictive practices
  • Health conditions needing nursing oversight alongside disability support
  • Participants other providers have declined

If you are unsure whether something sits inside our scope, call and ask. A five-minute conversation is cheaper than a referral that goes nowhere.

Why AHPRA registration matters to you

AHPRA is Australia’s independent regulator of health practitioners. Our supervisors and team leaders are registered with it, which gives you three practical things:

  • An independent complaints pathway for your participant, separate from us and separate from the NDIS Commission.
  • Clinical standards that do not drift. Registration is conditional on mandatory ongoing education.
  • Formal qualifications behind complex decisions. Mental health nursing and allied health training, not experience alone.

You can verify any practitioner yourself on the AHPRA public register. We would rather you did.

How to refer

  1. Send it through. Use the referral form — about five minutes — or call 0410 560 769 and we will take the details.
  2. We acknowledge within 24 hours. You hear from us either way.
  3. We contact the participant within 48 hours and arrange a free conversation.
  4. You get updates. Regular, with the participant’s consent, plus monthly progress reports once support starts.

What helps us move fast

None of it is compulsory, but the more you can give us up front, the quicker we can act:

  • Participant name, contact details, and the best way and time to make contact
  • Any communication needs or preferences
  • Funding type — NDIS, DVA or private — and how the plan is managed
  • Plan dates and which supports are funded
  • What is happening now, what needs to change, and how urgent it is
  • Any risks we should know about

Referral information includes health information, which the law treats as especially sensitive. We share it only with people who need it to deliver support, and we ask for the participant’s consent before sharing it with anyone else — including you, as the referrer. Read our privacy policy.
